Ground cover is slowly being killed from the edges. The ground looks to be tilled up around the areas that are dieing. I have found some small worms 1" or so long but don't know what they are how to stop them.

Welcome MCGardener. Maybe cutworms? Here is someone else with a similar problem and the suggestions for a cure.

And I can't find directions to link for you. But you need to have your pictures on a hosting site like Photobucket. Then when your picture is there you can add the img link here. Make sure your pictures are a size allowed, 640 by 480.
